Hot Work Permit

Please fill out the following Hot Work Permit form entirely.

This hot work permit is required for any work involving open flame, or producing heat and/or sparks outside a designated hot work area, within 25 feet of a structure, or if the precautions in designated or outside area cannot be followed.

Hot work includes welding, cutting, grinding, burning, sodering, or other work that produces a source of ignition. Hot work permit is good for one day only.

Notify immediate supervisor and location Manager-In-Charge (MIC) of intended hot work.

Ensure hot work equipment is in good condition.

All flammable liquids, oily deposits, removed from area (>35 feet)

Remove combustible material within immediate area of hot work or protect combustible material (including structure) within immediate area of hot work. Protection could include shielding with welding blankets, slag curtains, dampening or wetting material with water, etc.

Ensure fire extinguisher (s), water hose(s) and/or other fire extinguishing agents are immediately available in the hot work area.

All roof, wall and floor openings are covered.

Continuously watch for fire threats during hot work and immediately report any concerns to immediate supervisor and location MIC.

CALL 911 IF FIRE IGNITES AND CANNOT BE CONTROLLED.
